Tire Rack will mail a redemption form (also available here) and a copy of the invoice to your billing address within 1-2 weeks. You will need to complete this form and submit it along with the actual invoice (website order placed page and order/shipment confirmation emails are not accepted). An additional invoice copy will also be shipped with your order.

To receive the manufacturer’s rebate you must send a copy of the invoice and the completed official redemption form to:

Keep the original invoice for your records. Mail-In Rebate for a Goodyear Visa® Prepaid Card valid on purchases between October 1, 2012 through December 31, 2012. Redemption materials must be postmarked by January 31, 2013. One Mail-In Rebate for a Visa Prepaid Card per qualifying purchase. Void where prohibited. Offer available at participating U.S. Retailers. Offer valid only for U.S. residents. (Goodyear interprets “U.S. residents” as a billing address in the 50 U.S. states and D.C. including APO/FPO addresses). Not valid with any other offers.

A Visa Prepaid rebate card will be sent the address provided on the redemption form. Allow 6-8 weeks for Prepaid Card delivery. Goodyear reserves the right to substitute a check of equal value in lieu of a Prepaid Card at its discretion. Fraudulent submissions will not be honored and may be prosecuted. Sponsor is not responsible for noncomplying Rebate Submissions or for lost, late, illegible, postage-due or undeliverable mail.
