To receive an $80 Bridgestone Prepaid Mastercard, you must submit (i) an official claim form online and (ii) an original or legible copy of the invoice for the qualifying Bridgestone tires (website order placed page and order/shipment confirmation emails not accepted for this portion of the offer).
